Effective immediately upon execution, [Participant] do hereby release and forever discharge Victoria Cumberbatch DBA adventuresOFcommunity, their agents, employees, successors and assigns, and their respective heirs, personal representatives, affiliates, successors and assigns, and any and all persons, firms or corporations liable or who might be claimed to be liable, whether or not herein named, none of whom admit any liability to the undersigned, but all expressly denying liability, from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever, which I now have or may hereafter have, arising out of or in any way relating to any and all injuries and damages of any and every kind, to both person and property, and also any and all injuries and damages that may develop in the future, as a result of or in any way relating to the following:

• Breathwork
• Screamwerk
• Tapping [emotional freedom technique]
• Reiki Therapy
• Fascia Release

Potential injuries arising from this form of holotropic breathwork may include but are not limited to:

• Dizziness
• Loss of consciousness
• Seizures
• Hyperventilation
• Tingling in hands, arms, feet, and legs
  
• Tetany
• Irregular heartbeat
• Muscle spasms
• Changes in vision
• Ringing in ears
• Emotional distress

It is understood and agreed that the terms of this agreement are contractual and not merely a recital. This Release may not be altered, amended, or modified, except by a written document signed by both parties.

Furthermore, this Release shall be binding upon the undersigned, and his respective heirs, executors, administrators, personal representatives, successors, and assigns. This Release shall be subject to and governed by the laws of the State of Colorado. This Release has been carefully read and fully understood by the undersigned. The terms have been explained to me and I am freely, knowingly, and voluntarily entering into this Release.

You agree to follow a breathing practice, which could affect change in your health (mental, physical, emotional or spiritual). By entering into this work, you are responsible for your own experience and your self-care following the sessions. adventuresOFcommunity is not responsible whatsoever for any health issues(emotional, physical, mental, spiritual) in connection with any programme or work delivered.

adventuresOFcommunity practitioner, Victoria, is not a licensed medical care provider and is not rendering personal medical advice or treatment. Always consult your doctor or health care provider. Please inform Victoria if you’ve had any form of mental illness or on antidepressants or other medication you feel I should know about. Please inform me if you begin taking any medication or stop taking any medication that is relevant to our work together. Please see contraindications. If deemed appropriate, please supply an emergency contact name and telephone number.

Age restrictions: You must be at least 18 years old or have acquired the consent of your legal guardian in order to purchase this service.

Confidentiality: All information disclosed during breathwork or any work carried out by adventuresOFcommunity LLC, including written notes, remains strictly confidential. Your personal information will not be shared without your consent unless required by law. If you have registered your interest in receiving updates, news and marketing information, you are free to unsubscribe at any time. Your privacy will always be respected and any personal communication between you and Inspirational Breathing will always comply with the United States and state of Colorado guidelines for best practice and the Data Protection Act.

Photography: We may take screenshots during sessions for promotional purposes on the website and in the newsletter. Please let us know if you would prefer not to feature in these photographs and we will ensure to remove or cover your likeness. This permission was requested in the form when you denoted the time [s] of your session[s].

Payment: By accepting these terms you agree to be responsible for full payment for the work delivered, regardless of whether you attend all the agreed and scheduled sessions. Once servicing has begun, no refunds will be issued. Should any issues arise in relation to the payment, you agree to discuss this in advance in writing or via phone.

Cancellation Rights and Refunds: The service, once commenced, cannot be transferred or exchanged.

Once you’ve scheduled your date & time, you are unable to request a refund.

Termination: Services to be engaged in may be terminated or suspended, without prior notice or liability, for good reason. Refunds will then be negotiated. If any session is cancelled by the purchaser, an administration fee will be levied at the discretion of adventuresOFcommunity.
