Please complete this form to request assistance from Get Ready Hawaii for a child or children in need. Please feel free to complete this form multiple times if you are requesting for multiple children. Please be sure that this form is filled in COMPLETELY for each child, and that only one child's information is listed in each submission. Please note that all information is necessary in order to process your request.

School supplies will be distributed through community partners for those who's requests are made by youth serving professionals, or through a community distribution event for individual and family requests. If you have any questions about this process please feel free to email getready@getreadyhawaii.org.
