District Vaccination Interest Survey
As you may know, COVID-19 vaccines have become increasingly available to educators. Individuals who are interested in receiving a vaccine are asked to complete this form. Those interested in vaccination will be  placed on a list to be used for notifying staff of vaccination opportunities as supplies become available, based on the prioritization recommendations of MDE and/or MDH.

You are not legally required to respond to this survey or to provide the District with any information about your interest in the vaccine. However, if you neglect to inform the District that you wish to be vaccinated, you will not be placed on the list to receive notification as it becomes available.

If you inform the district that you want to receive the vaccine, the information you provide may be shared within the district to individuals who have a need to know the information in order to perform their job duties and to individuals and organizations outside the district who are authorized to receive the information in order to administer the vaccine in accordance with protocols established by federal, state, and local branches of government.

Prior to receipt of the vaccination, you will be asked pre-vaccination medical screening questions. If you choose not to answer these questions, the employer (or third party administering the vaccine) may decline to administer the vaccine.

Vaccination is optional and staff members have the ability to accept or decline if a vaccine is offered. NOTE: Employees who accept the vaccination opportunity will be required to travel to/from Mountain Iron, MN on personal time.
