If accepted, I understand that I will need a Illinois Sales and Use Tax permit.

MANDATORY SET UP RESPONSIBILITY
I understand that I am responsible for my entire setup (including tents, tables, chairs, lighting, and extension cords).

APPLICATION SET STATUS
I understand that I will only be contacted, if I have been accepted. Acceptance is not guaranteed. The Black Girl Butterfly Convergence / Inaugural Rites of Passage Conference does not send decline emails or application status updates to applicants.

Mandatory General Liability Insurance
If accepted, I understand that I will be required to obtain General Liability Insurance for the date of the event and Project Butterfly, LLC to vendor’s insurance policy as an additional insured.  Proof of this insurance in the form of a Certificate of Liability (COI) will need to be submitted to Butterfly Convergence Conference. Submission instructions will be sent with acceptance email.
