I understand that the staff and volunteers of St. Martin's Lutheran Church have done what is reasonable and appropriate in organizing a safe and secure environment for my child. I understand that Staff and volunteers have made arrangements to maintain proper safety standards for my child to participate in the week's activities.

I understand there are risks of play that may result in accidents or injuries. As such, I accept these risks of accidents or injuries to my child. I hold harmless and fully indemnify St. Martin's Lutheran Church, the staff of St. Martin's Lutheran Church, and the volunteers for Vacation Bible School from claims or damages arising from my child(ren)'s participation in Vacation Bible School activities assuming reasonable and appropriate safety measures have been implemented.

I give staff/volunteers permission to administer first aid, as well as make judgment upon necessity of a hospital visit, if such a situation arises.  

I release St. Martin's Lutheran Church and all of its entities from liability for any claims by me or any third-party in connection with my participation or my child(ren)'s participation
