BASA Online Summer Health/PE Registration
If your student desires to participate in our blended Health & Personal Fitness course this summer, we ask that you complete this form by March 31, 2022. This course will include both in-person sessions as well as online sessions. Attendance is mandatory - no exceptions. If you miss any of the sessions, you will not receive credit.

If your child currently attends BASA, the course fee of $60 can be paid via MySchoolBucks. You are also welcome to send cash or a check with your child to school so they can submit it to the front office. If your child does not attend BASA, you may pay at the BASA front office by cash or check. There are NO REFUNDS.

Please note, every student must attend every in-person and online session and complete all assignments of the course in order to receive the full unit. We look forward to supporting your child's health, fitness, and wellness this summer!

Agreement
This course will start on June 6th and will end on June 17th. This is a blended course which means it will have required in-person sessions and required synchronous online sessions. Coursework will be submitted in Google Classroom.

By submitting this application and paying the $60 fee, I understand that my child is committed to participating in the course and refunds will not be available. In order to receive credit, I understand that my child must attend ALL in-person and online sessions, complete ALL of the required assignments, and receive an overall passing grade in the course. This final grade will be entered on my child's permanent transcript upon completion.

I am also aware that if my child participates, they must attend all sessions. Virtual sessions will require students to have cameras turned on in order for the instructor to monitor participation. It is my responsibility to ensure that the proper technology is available for my student. Neither BASA nor BCSS will provide devices for my child to participate in this course.  

No exceptions will be made regarding attendance.

Rising 9th grade students are assigned Health and Personal Fitness as one of their eight courses for their freshman year. Because your child will be taking Health and Fitness over the summer, this will open up one additional course for them to take next year. What course would you like them to take instead of Health and Fitness?
