Quilt Selection for LexSeeHer Digital Quilt
This is the right form to complete to honor someone special in the LexSeeHer digital quilt project!
This is a one-of-a-kind gift. LexSeeHer will email you (and/or your recipient)!

When you receive your digital image, you can print it, frame it, make it into a mug.

You will need to select a square from the three Founder's patterns: Gift Box, Golden Gate, Rocky Mountain Puzzle.

You can also select a greeting for the gift email card that LexSeeHer can send directly to a recipient!

Remember to donate!  Donors who contribute $100.00 (or more) to LexSeeHer help bring the monument proposal forward and receive the opportunity to complete one digital quilt square. We will begin working on your square within 48 hours, and will await confirmation of your donation before sending.  

Optional: you are welcome to share information about your honoree through this form if you would like it to be included in our archive.

Name of the woman you are honoring, as you would like it to appear on the quilt square. *
Your name as you would like it to appear on the quilt square. For example, some people prefer their full name, some prefer initials, some prefer a first name only.  (If you do not want to include your name, tell us "honoree only"!) *
Optional: Please share information about the woman you are honoring, as you would wish it to appear in any publicity about the quilt. This is optional. You are welcome to include just the person's name. As we work to make women's contributions visible, we welcome the information you wish to share. Please limit your entry to 350 words. This information will remain in the LexSeeHer archive.
