Dear Sir/ madam,

How nice that your son of daughter likes to join our hockey club for two free practices.

Your child will be placed in a team suitable for his/her age. This way he/ she will join a regular hockey practice and experience how much fun it is to play hockey with a friend. Registration is mandatory and is possible by filling out this form.

After completing this form you will receive a confirmation E-mail. In this mail you will find information on with whom you will meet at our hockey club on the day and time of practice, what you have to bring and what we will provide you with.
