If you would like to have library materials delivered to your home please complete and submit this form to be eligible for home delivery service. You must live within the Viroqua Area School District boundaries. Any materials within the Winding Rivers Library System and Interlibrary Loan are available for delivery. You may call 637-7151 ext. 6 or utilize the website mcintoshmemoriallibrary.org to put library materials on reserve.

Responsibilities of Home Delivery Participant

Someone will accept materials upon delivery if I am not able to, they will not be left out of doors/exposed.

I will notify the library of any change of address.

I will notify the library if I choose to discontinue home delivery service.
