Please fill out the following form to register your child/children for the Master Club program at South Side Baptist Tabernacle.  Master Club is on WEDNESDAY nights from 6:45 PM - 8:30 PM, and the first night of Club is on WEDNESDAY, SEPTEMPER, 14TH.  In order to register for Master Club, the child has to be at least 3 years of age by September 14, 2022 and up to 6th grade.  We do have a teen program called ProTeens where your child can join from 6th-12th grade. On your first night of club, the parent(s)/guardian(s) will be asked to verify the information we have collected, and a signature will be requested.  A "Photographic Consent" form will also be provided to you; with your signature this form will allow South Side Baptist Tabernacle to take pictures of your child, and use them for promotional purposes, including our website (ssbtonline.org), on our Facebook Page (Facebook.com/SSBTYPSI), and on our Instagram Page (Instagram.com/SSBTYPSI).  *Declining to sign the "Photographic Consent" form will NOT affect your registration for Master Club.

Section 2 is for parent's Information required for registration.
Section 3-9 are individual child registration forms. (If you only have one child, just fill out Section 2 and Section 3 then skip to the end to submit.

*PLEASE NOTE:  You must fill in the Allergy section.  If your child does not have any allergies please write in 'none', thank you.
