Please Note:

1. Scholarships will cover part of the registration fee.  The rest will be due at registration.  
2. Students MUST be part of the West Des Moines Community School District.
3. Each student can only have one scholarship per semester (fall, spring, summer).
4. If the student can’t participate for some reason, another request can be made.
5. Fill out this form for one student at a time, each student needs their own form.
