Must be submitted by 3 p.m. the day of the game to reserve a spot.  
Please list each sports pass holder NAME from your family who plans to attend EACH game.
This may be submitted multiple times so feel free to update it as needed throughout the season.
IF ATTENDANCE RESTRICTIONS ARE EVENTUALLY LIFTED BY THE GOVERNOR AND/OR IHSAA YOU WILL NO LONGER BE REQUIRED TO FILL THIS OUT; HOWEVER, SPORTS PASSES WILL ALWAYS BE SCANNED AT EACH EVENT FROM THIS POINT FORWARD.
