This form is for persons who need documents from Nuview Union School School for verifying to the Department of Homeland Security - U.S. Citizenship and Immigration Services that they came to the U.S. before their sixteenth birthday.

What NUSD can provide:
* Dates of attendance for students who attended an NUSD school in any grade kindergarten through 12th, and who were born after 1992.
* Transcripts may be available for students who attended Mountain Shadows Middle School and/or Nuview Bridge Early College High School, and who were born after 1992.

What NUSD cannot provide:
* If students attended another public school, charter school, or private school, they will need to request documents directly from those schools.
* If students received their General Equivalency Diploma (GED), they will need to request these records from the state in which the diploma was issued.

NUSD attempts to provide student records as quickly as possible, however, it can take up to ten (10) business days. We will also contact you if additional information is necessary to process your request or if there is a delay.
