INSURANCE

Parent/Guardian states and agrees that the player is adequately covered with medical insurance elsewhere and imposes no fault on this Association.

REFUNDS
There will be NO refunds issued for tryouts.

WAIVER OF LIABILITY, RELEASE, ASSUMPTION OF RISK AND HOLD HARMLESS AGREEMENT

The undersigned Parent/Guardian on behalf of themselves and the minor child/children participant ("participant") agree: (1) to indemnify and HOLD HARMLESS the Chesterfield Hockey Association and its Board, along with the leagues in which they belong or subscribe, the officers, the coordinators, the managers, parents or any other individuals including those who transport them to and from practices/games ("the Releasees") for any damages or injuries to the child as a result of their participation in the Chesterfield Hockey Association and its programs or events; (2) to exempt, waive and release from liability for personal injury, property damage and wrongful death, including if caused by negligence (including the negligence, if any, of the Releasees) the Releasees and hereby do waive, release and relinquish any and all claims for liability and cause(s) of action, including for personal injury, property damage or wrongful death occurring to the participant, which arise out of participation in Chesterfield Hockey Association events, member team activities, the sport of ice hockey, and/or activities incidental thereto, whenever or however they occur and for such period said activities may continue, and by this agreement any such claims, rights, causes of action that participant may have are hereby waived, released and relinquished. Participant and the participant's parent/guardian acknowledge, understand and assume all risks relating to ice hockey and any member team activities and understand that ice hockey and related team activities involve risks to participant's person including bodily injury, partial or total paralysis and death, and damages that may arise therefrom and that I/we have full knowledge of said risks.

The undersigned Parent/Guardian, participant and/or registrant acknowledge and agree that in order to communicate with members, prospective members and participants, the Chesterfield Hockey Association and/or its Board members, officers, agents and coaches may, from time to time, send out information to individuals via fax, email or other means. By registering below, registrant acknowledges, agrees and consents to receive communications in such manner.

THE UNDERSIGNED AGREES NOT TO FILE ANY SUIT IN PURSUANCE OF ANY RIGHTS THEY MAY HAVE FOR ANY CLAIMS OR RIGHTS HEREUNDER OR OTHERWISE.

Electronic Signature:

Parent and/or Player authorizes the Chesterfield Falcons Hockey Club and/or its employees, assistants, or sub-contractors to photography or film/video Player and or Parent and to permit the use of display of said photographs or film/video in any publication, multimedia production, display, advertisement or world wide web publication. Parent and/or Player agree that the Chesterfield Falcons Hockey Club may use names, likeness, or biographical information. Parent and/or Player releases and forever discharges, indemnifies and hold harmless the Chesterfield Falcons Hockey Club, its employees, coaches, managers, agents and other Parents from any and all claims, losses and damages which may be suffered or incurred in connection with the use of said photography or film/video, images including by not limited to, any claims for invasion of privacy of defamation. I give this consent with no claim for payment or commission.

Tryouts will be $50. By agreeing below, you will be required to supply payment at the Check In Table on Day ONE of Tryouts. This may be in the form of check, cash (exact amount) or credit.
