Please complete the below form if you are enrolling a Pre-school student to the Smithton R-VI School District. After you complete the form you will be contacted by a district employee with further instructions to complete your child's registration.  **Completing this form does not guarantee enrollment. We are honored that you chose to enroll your child in the "Lil' Tigers Learning Center"(TLC). We are proud to serve you and provide you a fun and enriching first experience for your child. Before your child begins, we will need the items listed below to register your child.

- Copy of Birth Certificate
- Current immunization record
- Residency document (current electric bill, bank statement, lease/rental agreement)
- Social Security Number


YOUR CHILD MUST BE INDEPENDENTLY POTTY TRAINED to attend preschool.

AM CLASS is from 8 am to 11 am Monday-Thursday (preferably 3-year-olds)
PM CLASS is from 11:45 am to 3 pm Monday -Friday (preferably 4-year-olds)
