I/We, the Parent(s)/Guardian(s) of [child], give them permission to attend offsite youth events throughout the summer of 2022, as organized by Cold Lake Community Church and Harbour Light Alliance Church, understanding that the location of these events may be subject to change. 
I/we, named above, undertake and agree to hold blameless the ministry staff at Cold Lake Community Church, its Pastors, and Board of Elders from and against any loss, damage or injury suffered by the participant as a result of being part of this youth event or transportation to and from the event. In the unlikely case of an emergency, I authorize the ministry staff/volunteers of Cold Lake Community Church to sign a consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedure for the participant named above. Parents/guardians will be contacted immediately in case of any emergency regarding the health of the participant.This consent and authorization is effective while participating in or travelling to youth events both on or off church premises throughout July and August of 2022.
