I hereby declare that I have fully read and understood the terms and conditions within the Melbourne Budokai COVID-19 Return-to-Training Policy and Procedures Manual and will uphold them to the best of my ability. I consent that despite the measures put in place, there is still a risk of contracting COVID-19 at the dojo. I understand that my contact details will be collected by the committee for contact tracing purposes only, and maybe forwarded to the relevant government authorities if required. I also understand that if I breach the terms of the document, the committee has the right to refuse my continuing participation in training until it is deemed safe to do so. *
