In order to inform ELC staffing, we ask that you complete this form to indicate how you intend to use your ELC entitlement. Please note, your entitlement is available to you, but in the interest of providing flexibility, you are not required to use the full entitlement as this is not a mandatory service.
Should you wish to make any changes to your attendance pattern, we ask that you give us 4 weeks notice in order to plan accordingly.
