This Google Form is to be submitted by the Scheduling Supervisor who is due for their Performance Review (you) prior to your In-Person Performance Review. The Scheduling Director will also complete an employee evaluation form. During the In-Person Performance Review, you and the Scheduling Director will discuss where your evaluations align and where adjustments or further development would be beneficial.
You will be evaluating yourself on:
- Your exhibition of Forest Family Dentistry's Core Values;
- Your Scheduling Supervisor Technical Skills; and
- Your Scheduling Supervisor Soft Skills.
Additionally, you will share the professional and personal goals you want to achieve over the next year. At the end of your In-Person Performance Review, you and the Scheduling Director will agree upon expectations for the following year to help you achieve your goals.
