Transportation Plan Form 2022-23
ALL students NEED to have a Transportation Plan Form for 2022-2023 School Year on file in the Transportation Office whether or NOT transportation is needed to be provided to AND/OR from school.  

If you move over the summer, please be sure to update your address in the parent Oneview, no later than August 8th to ensure your child has a bus at the correct location, on the first day of school and to receive important communications from the school.

If you are attending UVCC, your form will need to be turned in by August 1st to ensure your child has a bus prior to the start date of August 18th.

**ONLY**If your child needs transportation to AND/OR from a 2nd court-ordered shared parenting address you will need to fill out our Shared Parenting Waiver and Calendar Forms. You will need to provide your Court-Ordered Shared Parenting paperwork for us to make a copy for our records.  ALL Forms are due by August 8th, 2022, to ensure we have the time to assign and send out communications to parents before the start of the school year.  Contact our office and we will send you the links for those forms OR you can come to the Transportation Office to fill them out.
