Please use this questionnaire to help Academia Tutoring place your student in the appropriate group of students that aligned with your students needs. Students will be placed in groups of 3 (students) according to their academic level and focus subject area. Each virtual or online session will be 60 minutes in length. Each student is responsible for $15 per session. In order for students to gain the most success  from our services,  there is a minimum commitment of 2 sessions per week. Students may be placed in a different group as needed. If you already have your group of students prepared please be sure students are about the same academic level and require the same core subject area. Should you have any questions feel free to leave them in the comment section.

The completion of this form does not guarantee any student will be immediately placed in a small group. All small groups will be set up on a first come first served basis as tutors are available for specific grade level and subject area of need. Someone from Academia Tutoring will contact you to confirm space availability, discuss scheduling, and complete invoicing. Academia Tutoring commits to offering quality tutoring service for the specified focus area in the time frame paid for. It is our policy that students put forth 100% effort and attention for the entirety of the time service is rendered. In the event, students are late, leave early, or become disconnected or not focused, full payment is still required and any prepaid monies will not be refunded. In the event a student is unable to attend any tutoring sessions for any reason, the student will have 7 days to make up that session or forfeit that session and forfeit all fees paid for that tutoring session. Should any student disrupt the learning of ANY student during their in person or virtual tutorial session, the disruptive student may be removed from the session. In addition, the student could possibly forfeit any other opportunities to join further academic sessions with Academia Tutoring.
