The receipt id should have been printed on the receipt we emailed you when you purchased the tags.   It should look like Receipt # 1234-5678. If you don't have the receipt leave blank.

Enter the amount you paid.
Enter Binbox Tag numbers. 
Enter one or more Binbox Tag numbers.  These should be printed on the tags you purchased.  They look like 123-4567-8901.  All these tags will be disabled - this prevents misuse of any of the tags you purchased.  If you don't have these numbers  leave blank.
Enter the reason why Binbox tags did not work for you.
We know our Binbox tags did not work out for you.  Any information you provide will help us improve our tags and experience so that we can get you as a customer in the future (we hope)!
