Please do not fill in this form until you have accounted for ALL volunteer positions for your team.  If a change must be made later in the season you can inform your Division Director and we'll get the information updated.

The four team volunteer positions requiring registering as an AYSO volunteer are Head Coach, Assistant Coach, Team Manager and at least one Referee position. Instructions for how to register as an volunteer can be found on www.aysoregion4.com.  

Other required/strongly suggested volunteer positions Goal set-up/take-down, Team Sponsor, Rater of the Opposing team (10U-14U).  Additional volunteer positions may include but are not limited to Team Banner, Team Party, and Team Safety.
