This form is to be filled out by students that would like to have their extenuating circumstances around absences reviewed by the attendance committee for the 2019-2020 school year. After your request is submitted, the committee will meet regarding your request.

Students and Student's Principal will receive an e-mail following the meeting with results of the student's request.
