https://resources.finalsite.net/images/v1634231265/parklandsdorg/wkxy4wz7xie3h463ujkq/StudentLeaveofAbsenceForm2020.pdf If a student is scheduled for a study hall at the start and/or end of the day, under certain conditions they are eligible to arrive late/leave the building early instead of attending their study hall(s). This is NOT an automatic right for all students; rather, it is a privilege granted to the student by the administration and may be revoked at any time due to poor citizenship, failure to comply with the guidelines of Late Start/Early Leave, and/or unsatisfactory academic performance.
Several guidelines must be followed in the late start/early dismissal procedure:
1. Students who are passing all subjects, have good attendance, and who do not have discipline problems are permitted to apply for late start/early dismissal.
2. A Request for Late Start/Early Dismissal Online Form must be completed by the student and his/her parent/guardian before the student may utilize the late start/early dismissal privilege.
3. Students who take advantage of late arrival may not be on school property until ten minutes before class starts and must report to class on time.
4. Students who take advantage of early dismissal must leave the school building and campus immediately upon their dismissal from their regular class of the day.
5. All requests are reviewed and approved by PHS administration.
Please screenshot and/or take a picture of these guidelines so that you have these for reference in the future.
Each student and their parent/guardian must complete this form. Only one student per form is permitted. Families with multiple students at PHS must complete a form for each student.
