The NHS definitions of ‘clinically extremely vulnerable’ and ‘clinically vulnerable’ have been included within the survey. If you are unsure if this applies to your child, please contact your GP or the medical professionals involved in your child’s care.

We recognise the sensitivity of the information we are requesting. Please be assured that a restricted group of staff will have access to the confidential information you supply. This group will comprise the Principal and members of the senior leadership team, pastoral and SEND staff including Learning Managers, the Inclusion Coordinator, SEND Officer and Attendance Manager, and the multi-academy trust’s Executive Team.

It may be necessary for a staff member to contact you to discuss your responses. We will do so using the contact details held on your child’s record. If you have any questions about the survey, please contact the academy.

The survey only needs to be completed for students who joined the school in September 2020. If you have more than one child at school, please complete a separate survey for each child. Surveys should be submitted by Friday 16th October 2020.
