By filling out and signing on this form:
1) I hereby give my approval to enroll the student on the registration form to Lone Star Chinese School.
2) I hereby give my permission to the student/students who on the form to participate in all school activities.
3) I hereby release Lone Star Chinese School, its teachers, volunteers, guests, other parents, and students from liability for any illness, injury, misadventure, harm of any kind suffered because of participation in Chinese school camps, classes, and all activities.
4) I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that above student/students and I may be exposed to or infected by COVID-19 by attending activities in Lone Star Chinese school. I further acknowledge that Lone Star Chinese School will not be liable if the student on the registration form and myself or other family members are infected by COVID-19.
5) I hereby give my permission to Lone Star Chinese School to act as guardian if I cannot be contacted in the event of an accident or medical emergency involving my child/children. Also, in the event of emergency, the school has my permission to obtain medical treatment for the proper care and well-being of my child/children, including ambulance service. All the cost is my responsibility.
6) I hereby give my permission to Lone Star Chinese School to take photos and videos of student on the form. The photos and videos may be released to the school, or other related flyers, websites, newsletters, weekly bulletin board, school parents, etc.

Monday to Friday: 8:30 am - 12:00 pm
All-day Camp:
Monday to Friday: 8:30 am - 4:00 pm
Lunch and Lunch Break 12:15-1:15pm: bring your own lunch or snacks, water or fruit juice; no soda
Special Time:
Field/Outdoor Day: Wednesday morning (May make an indoor field day, eg. Camping night if outside is too hot)
All about Food: Friday afternoon

Camp Fees:Register before April 30th:
Reg. fee: $20.00 per student; $30 per family
Morning Camp fee: $175; sibling $170.00 per week
All-day Camp fee: $255.00 ; sibling $245.00 per week
After May 1st:
Add $20 per week for full-day camp and $10 for the morning camp
Extended care: Morning Camp 12:00-2:00pm $10/hour;
Full-day Camp 4:00-6:00 pm $10/hour
Chinese Heritage Camp Weekly: $225.00
Daily (Day 1 - Day 4): $60.00
Daily (Day 5): $65.00
Register after May 1st: Add $20.00 weekly; $5.00 daily
