This Request for Financial Assistance Form is to determine eligibility for Great Hearts to waive or reduce the following cost on behalf of your child for the 21-22 school year: Early-bird registration for after-school care, school supplies (Edukits), uniform vouchers, and summer programs. Please fill out this form for each of your children. (Please note: The eligibility for Free or Reduced Lunch, official after-school care and/or athletics discounts are determined officially through a separate application that will be available in late July.)
