Registration Waiver, Medical Release, and Drop Off/Pick Up Responsibility Form

I, the undersigned, hereby acknowledge and agree to the following terms and conditions set forth by Seattle Persian School:

Registration and Attendance:

·      I understand that registration is complete only upon submission of the required registration form and payment.

·      I acknowledge that Seattle Persian School reserves the right to cancel any class if it does not reach the minimum number of registrants. In such cases, a full refund will be issued.

·      If a class is canceled for any reason, Seattle Persian School will make every effort to provide makeup options, which may include substituting teachers or offering online classes. If makeup classes are not feasible, the tuition for the canceled session will be refunded to the customer.

·      I understand that refunds will not be issued after the first session of each quarter, regardless of the reason.

Medical Release:I grant permission for any necessary medical attention to be administered to my child/student, in the event of an accident or injury, and I will assume all costs associated with any medical care.

Drop Off/Pick Up Responsibility:

·      I understand that it is my responsibility to drop off my child/student at the designated location and time specified by Seattle Persian School, and to ensure that they are picked up promptly after each class or activity.

·      I will notify the school in advance if someone other than myself will be picking up my child/student.

Media Release:I grant Seattle Persian School permission to use photographs or videos of my child/student for the purpose of promoting the school on its website or other social media platforms. I understand that these images will be used in a culturally sensitive manner and will not reveal personal identifying information.

By signing below, I confirm that I've read, understood, and agree to adhere to the terms during my child/student's enrollment at Seattle Persian School.
