Informed Consent Form
My name is Vanessa Wright. I am a doctoral student at Northcentral University. I am conducting a research study on traditional face-to-face learners who used online education, during Covid-19, as a medium to advance.

The intent of this research study is to consider what students experienced when they were converted to online math courses, and to gain a thorough understanding of their opinions and approaches. I am completing this research as part of my doctoral degree. Your participation is completely voluntary.

Participation in this research includes responding to a survey or an online interview using Skype, Google Hangouts, FaceTime, Watsapp or any other online video conferencing software, and should take approximately 15 minutes.

I am seeking your consent to involve you and your information in this study. Reasons you might ​not want to participate in the study include you might think that your academics might suffer, by participating. Reasons you might want to participate in the study include you might want to build your own skills for graduate or a professional school. An alternative to this study is simply not participating. I am here to address your questions or concerns during the informed consent process.

Activities
If you participate in this research, you will be asked to:
1. Choose to fill out an online survey OR participate in a virtual interview using Zoom or any other video conferencing software.
2. Answer about seven questions about becoming a math eLearner.

Eligibility
You are eligible to participate in this research if:
1. You are a first-year community college math learner that was enrolled in a traditional face-to-face math course that was converted to an eLearning course due to Covid-19 Pandemic.
2. Corresponding mathematically using online tools is necessary to complete your coursework.

You are not eligible to participate in this research if:
1. You did not complete course work using math eLearning technologies.
2. You are under the age of 18 years

Risks
Research on learners’ experiences exposes the participants to minimal risk. To decrease the impact of these risks, you can skip any question, and/or, stop participation at any time. Certain private information may be collected about you in this study, such as your name, your email address, and your age.

I will make every effort to protect your private information, including using pseudonyms of famous authors to protect participants’ information. Even with this effort, there is a chance that your private information may be accidentally released. The chance is small, but does exist. You should consider this when deciding whether to participate.

Benefits
If you decide to participate, potential benefits could include your contribution to the math community and your impact on future students. No other personal benefits will be applied for your participation.

Confidentiality
The information you provide will be kept confidential to the extent allowable by law. Some steps I will take to keep your identity confidential are using pseudonyms of famous authors to protect participants’ information.

The people who will have access to your information are: myself, and/or, my dissertation chair, and/or, my dissertation committee. The Institutional Review Board may also review my research and view your information.

I will secure your information with these steps: using encryption on my computer.

I will keep your data for 7 years. Then, I will delete electronic data and destroy paper data.

Voluntary Participation
Your participation is voluntary. If you decide not to participate, or if you stop participation after you start, there will be no penalty to you. You will not lose any benefit to which you are otherwise entitled.

Future Research
Any information or specimens collected from you during this research may ​not ​be used for other research in the future, even if identifying information is removed.
