I have enrolled in Dance to Live Salsa dance classes. I recognize that the program involves physical activity.

I hereby affirm that I am in good physical condition and do not suffer from any
known disability or condition which would prevent or limit my participation in these dance classes.

I will not hold Paula Videla-Rodriguez, Jose Rodriguez or Dance to Live, liable for any personal injury or any personal property damage/loss, which may occur on the premises before, during or after classes.
