Healthcare Accessibility Provider Survey
This survey is intended for healthcare providers who serve people with disabilities. It was created by the Pennsylvania Health Access Network as part of a project funded by the PA Developmental Disabilities Council to improve access to healthcare for people with disabilities in Central PA. The data from these surveys will be used to develop trainings and policy initiatives.
