MEMBERSHIP CANCELLATION

All class membership passes require a 3 month FIT4MOM Greater NW Chicago Suburbs programs commitment. Cancellation prior to the 3 month commitment timeframe will result in a $60 Early Cancellation Fee (due to current climate we are waiving the 3 month commitment). The Membership Cancellation/Hold Request Form must be completed and submitted at least 14 days prior to the 1st of the month in which you wish to cancel your membership. Failure to submit this form within the requested timeframe will result in the monthly auto-charge for your membership on the 1st of the month. There are no refunds on your membership once monthly payment has been processed. If you wish to return to FIT4MOM Greater NW Chicago Suburbs within 3 months of cancellation you will not incur an enrollment fee. If you return to FIT4MOM Greater NW Chicago Suburbs after 3 months you will be charged an enrollment fee.

MEMBERSHIP HOLD

Monthly membership passes may be placed on hold for reasons such as medical conditions and maternity leave at no charge. For non-medical reasons, there is a $10 per month hold fee, which will be auto-charged on the 1st of the month of the requested hold. For non-medical hold requests, the Membership Cancellation/Hold Request Form must be completed and submitted at least 14 days prior to the 1st of the month in which you wish to hold your membership. Qualifying memberships can be placed on hold for no less than 30 days. A maximum of two holds are allowed per calendar year (Jan-Dec). After the hold period has ended, your original membership pass will resume, along with automatic monthly payments when applicable. There are no refunds on your membership once monthly payment has been processed.
