HOURS:
Monday to Friday: 9:15 am - 4:00 pm
Before Care: 8:30 am - 9:15 am (Advance notice needed)
After Care 4:00 pm - 5:00 pm ($10 fee & advance notice needed)

COST:
$90 Per Day (before Child Care Subsidy if eligible)

 TERMS AND CONDITIONS:
* A minimum of three days of bookings is required (exceptions can be made by contacting Rabbi Yitze).
* Once you have received confirmation via email your booking is confirmed. Cancellations after 1 Jan will still be charged at full fees.
* All excursions are subject to change. You will be notified on any changes and will not be charged if a change in booking is made as a result.
* If your child has a medical condition they will not be able to attend Chabad Bentleigh Holiday without the parent/guardian proving Chabad Bentleigh with the required information and medication.  
* If you wish to receive the Childcare Subsidy it is your responsibility to make sure you have fulfilled all government requirement by the onset of the program. If not you risk paying full fees
* The contact details, including all emergency contact details, listed on my child's Enrolment Form are up-to-date.
*  Parents agree to have their child excluded if their child’s temperature is above 38°C
* Families must immediately advise the Service if they develop any symptoms of the virus or receives a positive result of the virus whilst in isolation
* Any person who is displaying symptoms such as: fever, coughing, sore throat, fatigue and shortness of breath, must not attend our Service.

Authorisation for Excursion Form:
Our service has organized an excursion for your child to attend. All of the relevant details are provided below for your convenience. The service has conducted an Excursion Risk Assessment which is available for your review upon request. For your child to attend, you are required to read this information and complete the Authority for Excursion Form. To complete this Form, you must be listed on the child’s enrolment record as either their parent or an Authorised Nominee.

Reason for excursion: Excursions during vacation care are for children to develop an understanding of belonging to groups and communities and to learn to interact in relation to others with care, empathy and respect. The excursions create a fun learning environment with physical activities.

Transportation Details (to and from): The most convenient and safest route as determined by the bus driver at the time. They will be going by private charter bus.

Staff to child ratio: Will be a minimum of 1:15. Up to 40 children with 3 staff and 3 volunteers.

The Period the Child will be Away from the Service Premises Children will be off premises for approx. 3 - 4 hours (unless on a full day excursion).
