The first 5 families that register and pay will be accepted (up to 5 members per family)

TO: Indigenous Sport Council of Alberta (ISCA), (hereinafter collectively referred to as “the Sponsors”)I am aware that my participation in the 2021 ISCA Family Ski Event is at my own risk and that there are dangers and risks inherent in the exercise activities, including, but not limited to, head trauma, broken bones, muscle and ligament sprains/strains and cardiac and respiratory distress. I freely accept and fully assume all such dangers and risks and the possibility of personal injury, death, property damage or loss resulting therefrom. In consideration of ISCA permitting my participation in the event, I hereby agree as follows: THAT I knowingly and freely assume all risks of the event, both known and unknown, even if arising from the negligence of the Releasees (defined below), and assume full responsibility for my participation.TO WAIVE ANY AND ALL CLAIMS that I may have against ISCA and their respective directors, officers, employees, agents and representatives, and any volunteers in any way associated with the Challenge (all whom are hereinafter collectively referred to as “the Releasees”);TO RELEASE THE RELEASEES FROM ANY AND ALL LIABILITY for any loss, damage, injury (including death) or expense that I may suffer or that my next of kin may suffer as a result of my participation in the Challenge due to any cause whatsoever, INCLUDING ANY NEGLIGENCE ON THE PART OF THE RELEASEES; TO HOLD HARMLESS AND INDEMNIFY the Releasees from any and all liability for any property damage or personal injury (including death) to any third party, resulting from my participation in the event; and THAT this Waiver and Release of Liability shall be effective and binding upon my heirs, next of kin, executors, administrators and assigns in the event of my death. I have read and understand this Waiver and Release of Liability prior to signing it, and I am aware that by signing this Waiver and Release of Liability I am waiving certain legal rights (including the right to sue) which I or my heirs, next of kin, executors, administrators and assigns may have against the Releasees. RELEASE FORM I am the participant (or legal guardian of the participant is under the age of 18 years old) registering and hereby authorize the the Indigenous Sport Council of Alberta (ISCA) to potentially use the name and/or image of myself (or participant under the age of 18 years old) in its public communication materials. I realize that I may withdraw my consent in writing at any time by contacting the ISCA at iscaexecdir@gmail.com. In giving my consent via this electronic signature by typing in my name, I hereby release and hold harmless the Indigenous Sport Council of Alberta and their agents, employees, officials, representatives and contractors from any and all responsibility or liability for damage of any kind suffered in any manner whatsoever. I hereby relinquish any and all personal or proprietary rights I may have in connection with such use. I understand that I will receive no compensation should any photograph or video of me be used. *
