Tree Dedication Program - AJ Jolly Park

Jolly Park Community Development Council's tree dedication program encourages donors to honor, memorialize or celebrate a special person, place or event. At the same time, it improves the beauty and quality of AJ Jolly Park. Tree dedications can be done any time of year. We currently have trees available for dedication along the walking path near the main entrance to the Park. Tree dedications at the Park are $500 and include a dedication plaque. If Yes, please indicate the tree number from the map. Trees marked SOLD are not available. Trees 12 and 13 are Royal Raindrop Crabapples (flowering trees). Trees 1 and 9 are mature maple trees that were not newly planted (mature, nice shade trees).

Wording on tree dedication plaque (3-4 lines of text). Maximum of 26 characters per line. Jolly Park CDC will make very effort to meet the request of the donor with regards to tree plaque wording. First line of plaque is standard phrase: Tree dedicated to or Tree planted in memory of or Tree donated by. Please indicate first line standard phrase, followed by name of person, company or organization (2nd line), then 1-2 additional lines of text. *

Payment of $500 can be made to Jolly Park Community Development Council in two ways. Please indicate which payment method you will be using. *

Check made payable to Jolly Park CDC and mailed to 1565 Race Track Road, Alexandria, KY 41001 (Please indicate tree # and/or name of person on plaque)

Pay online with credit card at JollyParkCDC.org (Click Give, Donate, Other and indicate tree # or name of person on plaque in On Behalf Of section)
