I, the undersigned, grant permission for my daughter to participate in Grow Lead Inspire's Reignite Summit.

I understand that the organizers will take all necessary precautions to ensure the safety and well-being of the participants.
I acknowledge that I am responsible for, or give my child permission to travel to and from the Harlem School of the Arts for the summit.
I permit my child to be photographed or be part of the video that will be used for marketing, promotion, and advertisements.
I am aware that alcoholic beverages and/or illegal drugs are prohibited, and I have discussed this prohibition with my child. I understand that if my child is found in possession of these substances, they will be dismissed.
For medical emergencies, I authorize the medical team of this organization to take care of my child.
I release this organization from any and all liability for accidents or injuries to the child during the organization-related events.
