This form must be completed by a parent/guardian who is requesting an accommodation in accordance with the district Face Covering Policy. Please note that all students must wear a face covering in order to participate in the hybrid learning model. Students who are unable to wear either a face covering or agree to an appropriate accommodation approved by the District may need to participate in the full-time distance learning option (Note: special considerations will be given to students with a diagnosed developmental, medical or behavioral health condition).
