Flooding Survey 2016-2020
INTRODUCTION

This survey is about the financial and personal losses that individuals and families have suffered as a result of flooding. These losses include damage to property and belongings, work interruptions, housing displacement, and illness or emotional distress. Your answers should cover expenses and other losses you have experienced since January 1, 2016 as a result of flooding.
 
Your responses will lead to a better understanding of how your community is being affected by flooding, and help to put in place policies that protect residents against future flooding. If you have any questions about the survey, please send an email to info@impactcensus.org.

 
INSTRUCTIONS
 
You should only take this survey if flooding has affected you or your household personally or financially. By "your household," we mean people who live with you AND who are related to you by birth, marriage, partnership, or adoption.
 
As to property damage, this survey covers one property only. If you would like to tell us about more than one property, please take the survey again after submitting this one.
 
You should be at least 18 years old. Your participation is completely voluntary and you may withdraw at any time. All information you provide is strictly confidential, and no published research using data from this survey will identify you or your address in any way.
