I understand that my child’s participation in Dragon Wagon is voluntary and that my child and I are free to choose not to participate. I consent to my child’s participation in Dragon Wagon and understand that Linwood E. Howe Elementary School, CCUSD, Culver City Safe Routes to School, the Linwood E Howe PTA, and their respective employees, volunteers and agents will not be liable for any injuries or damages resulting from my child’s participation in Dragon Wagon. I, on behalf of myself and my minor child, agree to release, hold harmless and indemnify the aforementioned parties from any loss, cost, damage and/or expense of any nature which I or my child may have resulting, either directly or indirectly, from my child’s participation in Dragon Wagon/Safe Routes to School. Entiendo que la participación de mi hijo en Dragon Wagon es voluntaria y que mi hijo y yo somos libres de elegir no participar. Consiento la participación de mi hijo en Dragon Wagon y entiendo que Linwood E. Howe Elementary School, el CCUSD, Culver City Safe Routes to School, la PTA y sus respectivos empleados, voluntarios y agentes no serán responsables por lesiones o daños resultantes de la participación de mi hijo en Dragon Wagon.
