Photography/Publication Release:  I give permission for my child to be photographed or video-taped by school staff or local news organizations during the current school year. Their picture may be used in newspaper, television, stories and other promotional products.
Parents/guardians have the right to refuse use of student pictures, video or names for publication for school use.  This can be done by notifying the school office in writing.  Please note that during any “public” events at school, such as assemblies and performances, pictures and/or videos may be taken and distributed by those in attendance.

Field Trip Permission:      My child has permission to accompany his/her class on school sponsored field trips. I will be notified in advance when and where these trips will take place. I also give my permission for my child to ride the bus to any location in the district school related activities, assembly, etc. *
Art Work Release:  Time to time we have the opportunity to display student’s artwork in the community or have it published as part of the school’s Annual Report.  Please sign below to give permission to display your child’s artwork outside of the school building. I give permission for my child’s artwork to be used in the ways listed above. *
Family Handbook Acknowledgement: I understand that it is my responsibility to review the Family Handbook with my child for an overview of the rules and regulations.   *
