-This form is to be used in connection with your RETURN of poles that have been rented from Stacy for the 2024 Track Season.  

STEP ONE: Inspect your pole and take a picture of the Length/Weight/Flex markings (at bottom of pole)

STEP TWO: Fill out this form

STEP THREE:  OPTIONS available for return of poles

            OPTION 1: 4A/5A STATE MEET [Mountain View HS] - Stacy will be in attendance to pick up poles immediately after competition concludes will have a bag on the field

            OPTION 2: SDVC GYM: Friday, May 19th between 4:00-4:30pm

            OPTION 3: SDVC GYM: Saturday May 20th between 2:30-3:30pm

             OPTION 4: If you cannot make any of the above times, contact Stacy directly to arrange time and place drop-off [208-241-1234]

 LATE DROP-OFFS: Any poles not returned by 5:00pm on Monday after State Championships will be assessed a $50 late fee.
