Family Return to School Survey
Although the school district does not yet have a specific date to begin hybrid, in-person learning at our schools, we need families to take a few moments to assist us in planning for the return of students. The purpose of this survey is:
Identify students who will remain in remote learning, when we switch to our hybrid model.
Get family preference on pairing siblings/family members in learning cohorts.

There is a space provided for information for each child in your household. If you have more than one student in your household who will attend school when we move to hybrid learning, please also indicate your preference for pairing members of your household in the same Cohort or having them attend hybrid in different Cohorts.

Student Cohort Preference in Hybrid Learning Model
This model allows for in-person face-to-face instruction two days a week and three remote, live teaching days a week. In this model, students will alternate between in-person and remote live learning; but they will attend each class every day.  Students will be assigned by the school to be a member of Cohort A or Cohort B. Cohort A students will attend in person on Mondays and Tuesdays and then remote the rest of the week. Cohort B Students will be remote for the beginning of the week and attend in person on Thursdays and Fridays.

We are asking families who have more than one child in district schools to indicate whether they want their family members paired in the same Cohort or in different Cohorts. You do not need to answer this question, if you only have one child in school or if you are planning to keep your child in remote learning.

When schools switch to the hybrid learning model, our family would like our students to be placed:
