Hello District 203/204 middle school Parent! Thank you for taking this survey! This survey is from the Power of Choice at 360 Youth Services and we are hopeful that you can help us regarding a project for parents.

The purpose of this survey is to gather feedback from District 203 & 204 parents of middle school students in regards to how they best receive information.

The Power of Choice is a project of 360 Youth Services and works collaboratively with Districts 203 & 204. This campaign supports youth, parents and schools, focused on building resiliency and providing education to increase the number of students making healthy choices regarding substance use.
