Welcome to Channel5 Gamin's Annual Community Awards Nomination Form!

To nominate a member or creation for an award please first select the Award Category from the drop down menu and then enter the creator's name and the link to their workshop if the award is for the creator in general or  a direct link to the creation if the award is for a specific creation. We have also provided a space for you to provide the justification for your nomination for our consideration if you wish to use it.

To be counted as "this year" the nominated creation should either have been uploaded to the workshop or spotlighted on the Channel this year. It is NOT required for a creation to have received a featured spotlight on the Channel to qualify for nomination.

It is absolutely acceptable to nominate a creator or creation in multiple categories should they apply.

Everyone is welcome to nominate in every category and as many times as they wish!
