我（申請人）聲明所有個人數據都是真實正確的。我知道參加 Fitness Academy Asia Limited 的活動會使我面臨人身傷害的風險，並且我同意保持 Fitness Academy Asia Limited，其董事，員工和子公司，演示者以及來自此課程引起的任何和所有責任，包括但不限於肌肉拉傷、撕裂、拉扯、骨頭骨折、死亡以及任何及所有疾病，疾病或個人財產損失。

我（申請人）了解參加此課程所帶來的風險，並證明我的身體狀況良好。我也了解到在課程期間我可能會被錄像、錄音和照相，Fitness Academy Asia Limited 可能未經我事先同意就將我的圖像用於所有用途。我進一步同意所有註冊條件，包括但不限於不退款政策。

I (applicant) declare that all the personal data are true and correct. I am aware that my participation in the Fitness Academy Asia Limited's event (s) exposes me to a risk of personal injury and I agree to hold harmless Fitness Academy Asia Limited,  their Directors, employees and subsidiaries, the presenters and all the event staff from any and all liability arising from this course including, but not limited to, muscle strains, tears, pulls, broken bones, death and any and all illness, ailments or loss of personal property.

I (applicant) understand the risks arising from the participation of this course and attest that I am in sound physical condition. I also understand that I may be videotaped, audiotaped and photographed during the course and Fitness Academy Asia Limited may use my images for any and all uses without my prior consent. I further agree to all conditions of registration, including but not limited to, the no refund policy.
