Nemesis kids YouTube class registration
YouTube classes are prerecorded and our coaches will not be able to view the participants while taking part. This means injury and risk of injury are responsibility of whoever is supervising the child. This form is our registration form used to gain contact details for the participants parent or guardian. Completion of this form is required before you can be properly registered for our class.

1. I am aware Nemesis cheer and dance class like any other exercise can be strenuous. *
2. I am aware that it is my right to decide what elements of online class my child participates in and they can withdraw at any point. *
3. I understand it is my responsibility to ascertain if there is any medical reason why my child cannot participate in class.

4. I understand and accept that it is my responsibility to check the exercise area is safe from risk (e.g. slip and trip hazard)  as classes are prerecorded and cannot be viewed by coaches. *
5.I understand that cheer, like any exercise can cause potential muscle stiffness, soreness and other injuries/ health issues and fully accept these injuries are my responsibility and I am happy to proceed. *
6. I understand these classes are private and intended for the purpose of paying customers I do not have permission to share these online classes with anyone else and can confirm I will not do so. *
7. I give permission for Nemesis Cheer and Dance to store my information and use it contact me regularly to provide class videos and class information. *
8. I understand that because classes are prerecorded and the instructor cannot see child. It is my responsibility to supervise my child and contact emergency services if needed. *
I accept full responsibility for any injuries my child may occur during participation in Nemesis online Cheer class and waive the right to any claim against Nemesis Cheer and Dance and the person(s) of entity involved in anyway. I understand class is prerecorded and it is my responsibility to ensure safety of my child, the participant. I have read this informed consent waiver and release liability. Accepting all terms and conditions outlined above.

Indicate payment method and forward first payment at your earliest convenience to receive class video. You will not receive your class until we receive payment. Classes can be paid in block payments or weekly, (please use participant name as payment reference). Please note no refunds will be provided.
