HIGH SCHOOL DIRECTORS, PLEASE USE THIS FORM TO SUBMIT NOMINATIONS FOR THE 2023 MID-SOUTH HONOR BAND.

Please nominate a maximum of eight students (grade 9-12) in order of preference for the Mid-South Honor Band. Information should include students' name, grad, and instrument. If euphonium, please indicate treble or bass clef. If contra-bass clarinet, please indicate Eb or Bb. The evaluation should be a summary of the student' performance ability, accomplishmients, attitude, etc.  To avoid cancellations, please nominate only those students without conflicts who are committed to participating.
