Introduction
Dear Participant,

My name is Abigail McArdle and I am currently undertaking a Final Year Design Project at the University of Limerick, Ireland under the supervision of Dermot McInerney. The title of my proposed research is "A way to alleviate some of the expense and waste associated with competitive and professional dancing".  The purpose of this project is to explore ways in which the dance world can improve its sustainability by potentially lowering its costs and waste and increasing reusability and durability of dance costumes, uniforms and shoes. I hope to achieve this by gaining an insight into the lives and habits of dance teachers, dancers and dance parents primarily. This questionnaire and subsequent interviews, should you wish to be involved, will help to guide my research into a specific direction and to allow me to gain a full understanding of any problems faced by dancers, teachers and parents.

The results of this questionnaire will be completely anonymous. There is no need to use your real name when asked for a name as this is only for data collation. Your name or alias and any other information that could identify you will not be used in my final report. By consenting to participate in this questionnaire I would like you to complete the following short survey and answer the questions to the best of your ability. The more information that I can gather the more thorough my research can be. 

Based on the answers received in this survey there is a possibility that I will be carrying out more in-depth interviews. I would hugely appreciate it if you could indicate, on the consent form, whether you would be willing to be contacted further about this project by leaving your email and name. Consenting to further aspects of this research does not mean that you will have to participate later as it is always up to you, the participant, what you partake in.

Your participation is voluntary, and you have the right to withdraw at any time.  To participate in this study you must be over 18 years of age. Any relevant information collected will be compiled into my final research report and submitted to the University of Limerick.
