Thank you for taking the time to fill out this form.  We welcome feedback on ISTA Trainings, Festivals and Events, as well as feedback regarding our Core community members, including Faculty, Organisers and Apprentices.  

This is the place to share your suggestions, concerns, complaints, and appreciations.  If you are reporting an incident in which harm was done to you or someone else, please fill out an Incident Report.  

If you wish to share your feedback or report an incident anonymously, we invite you to contact Safe Mediation, an independent 3rd party, who will receive your feedback and act as a bridge between you and ISTA.

If your concern is regarding a specific member of Lead Faculty, this person will not be involved in the receiving or processing of your feedback.  It will be overseen by someone else in the Feedback Triad.
